Launch issue: the first 10 weeks

Every new psychedelic science paper, preprint and trial registration since late July: what mattered and what did not hold up.

1. Psilocybin prevented chemotherapy nerve damage in mice

Two doses of psilocybin before platinum or taxane chemotherapy stopped peripheral neuropathy from developing, across repeated chemo cycles, without weakening the chemotherapy against tumors. The authors trace the effect to nerve cells keeping their mitochondria moving along the axon.

2. A placebo-controlled psilocybin trial inside England's NHS

60 people with treatment-resistant depression received a single 25 mg dose of psilocybin or a placebo, both with preparation and integration support. At week 3, depression scores were about 10 points lower with psilocybin, and the difference held at week 6. 59 of 60 people completed every follow-up.

How much weight it carries: Randomized and double-blind, but small and designed as a feasibility trial. The authors call for a larger confirmatory trial. (Humans, RCT, n = 60, Peer reviewed)

3. On psilocybin, brain activity lines up with the surroundings

62 adults were scanned with fMRI and EEG at rest and during meditation, music and a film, before and on the day of psilocybin. Activity did not simply become disordered. It reorganized into patterns tied to the context, most strongly in people who felt continuous with their surroundings, a state the authors call embeddedness. The effect also tracked next-day changes in mindset.

How much weight it carries: The largest single-site psychedelic imaging dataset to date. Same people compared before and after dosing; this is about mechanism, not treatment. (Humans, n = 62, fMRI + EEG, Peer reviewed)

4. One mushroom makes both psilocybin and death-cap toxins

Galerina indica produces psilocybin and amatoxins, the toxins in the death cap. Mass spectrometry found both in the mushroom tissue, and its genome carries the genes for both. It appears to have picked up the psilocybin genes from another fungus through horizontal gene transfer.

5. Oregon's legal psilocybin services: first safety and outcome data

346 clients at 24 licensed service centers were followed for three months. Four people (1.2%, all first-time users) had reactions that needed medical attention. At one month, 92% said they had benefited. At three months, the share with moderate-to-severe depression was 60% lower than at the start, with similar drops for anxiety and PTSD symptoms.

How much weight it carries: No control group. The sample was mostly white, college-educated and well-off, so it may not reflect who would use these services more widely. (Humans, n = 346, No control group, Peer reviewed)

6. Zurich's real-world psilocybin results trail the trials

Switzerland allows limited medical use of psilocybin. Records from 19 treatment-resistant patients at the Psychiatric University Hospital Zurich, given one to four sessions, show depression scores falling from 31 to 20 on average. 33% responded and 22% reached remission, with no serious adverse events.

How much weight it carries: Retrospective, uncontrolled and small, and patients stayed on other medication. Response rates are below those in controlled trials. (Humans, n = 19, Retrospective, Peer reviewed)

7. Do you need the trip for the benefit?

In a mouse model of autism, one dose of psilocybin made the animals lastingly more social, even when it was given under anesthesia. A second mouse study found psilocybin still helped fear extinction when the 5-HT2A receptor, the one behind the trip, was blocked. A commentary in the International Journal of Drug Policy argues that removing the experience removes much of the therapy.

8. Chemists are redesigning psychedelics for safety

A PNAS team took LSD's four-ring core apart into nine simpler molecules, mapped which parts drive the hallucinogenic effect, and found two candidates with lower hallucinogenic and heart risk. A Nature Communications group used cryo-EM structures to design compounds that activate 5-HT2A while blocking 5-HT2B, the receptor linked to heart valve damage.

How much weight it carries: Lab and mouse work. These molecules are years away from human testing. (Lab + mice, Drug design, Peer reviewed)

9. Microdosing doesn't beat placebo in healthy adults

A meta-analysis of 24 studies with 3,681 people found no clear effect of microdosing on depression, anxiety or stress in placebo-controlled trials. Observational studies do report improvements, which the authors put down largely to expectancy and lifestyle.

How much weight it carries: Only two randomized trials (117 people) could be pooled, so the trial evidence is thin in both directions. (Meta-analysis, 24 studies, n = 3,681, Peer reviewed)

10. Early pilots in veteran PTSD, phantom limb pain and bipolar II

12 US veterans with severe, treatment-resistant PTSD received two psilocybin sessions with therapy. There were no serious adverse events, and 75% were in remission a month later. Small pilots also tested psilocybin in phantom limb pain (9 people, placebo-controlled) and bipolar II depression (14 people, open-label).

How much weight it carries: Open-label or very small. These results justify larger trials; they don't show the treatment works. (Humans, n = 12, Open-label, Peer reviewed)

11. 20 new trials registered

New registrations include psilocybin for methamphetamine use disorder, borderline personality disorder, nicotine craving and premenstrual dysphoric disorder, and a National Institute on Aging trial pairing psilocybin with cognitive training. Results were posted for psilocybin in depression and anxiety in Parkinson's disease.

12. Big Pharma and the FDA move

Eli Lilly agreed in July to acquire AtaiBeckley for $2.8 billion. In September, FDA officials set out the agency's updated framework for developing psychedelic drugs in the New England Journal of Medicine, building on its 2023 draft guidance.
